Select the Chapters tab.. 

Click on the plus button to add a new chapter Add Chapter button

Adding a Scene

You add scenes to a chapter 

Select the chapter 

Click on the Scenes section.

Click on the plus button to add a new scene Add Scene Button

Another way to add chapters and scenes is to use the storyboard.
